What Filler Really Does
Most modern fillers are made of hyaluronic acid (HA)—a substance naturally found in the skin that binds water and supports soft volume. When placed strategically, HA filler can restore contours that flatten with age, soften folds that deepen with volume loss, and refine features like the lips, chin, and jawline. The outcome depends far more on plan, placement, and depth than on any single product name.
Hyaluronic acid fillers are made with bonds that give them structure and lifting capacity. Over time, those bonds gradually weaken. It holds shape because it’s engineered to do so. Over time, that structure softens. The lift fades before the material fully disappears. Residual HA can remain in the tissue even after its lifting effect has diminished.
This is where many patients get into trouble. Repeatedly adding volume without reassessing underlying structure can lead to heaviness, distortion, or the “overfilled” look patients fear. More product does not equal better architecture.
At PCPS, we don’t chase lines with syringes. We restore support where it’s truly missing. Sometimes that means adding volume. Sometimes it means dissolving old filler. Sometimes it means choosing a completely different treatment.

Plan Before Product
At The Pittsburgh Center for Plastic Surgery, filler is never a “menu item.” Your consult is where we decide what actually needs to change—and what should be left alone. We assess proportions, skin quality, bone structure, and how you move. Then we map a plan: where volume is missing, where contour is needed, and where adding filler would create heaviness. You’ll leave with a written estimate and a clear explanation of why we’re choosing each area.
Material Matters
We use hyaluronic acid fillers from the Restylane® family and the RHA® (Resilient Hyaluronic Acid) collection. Both are advanced HA fillers, but they are engineered with different levels of flexibility and structural support.
Rather than choosing a brand first, we choose based on function. Some fillers are designed for lift and contour (ideal for restoring cheek support, refining the chin, or defining the jawline). Others are designed to flex naturally with facial movement, making them better suited for smile lines or areas that animate when you speak. Softer formulations are used for delicate refinement, such as lips or fine lines, where subtlety and movement matter most.
Structural options like Restylane Contour, Restylane Lyft, and RHA 4 provide support where projection and definition are needed. More flexible options like Restylane Defyne, Restylane Refyne, and RHA 2 or 3 are chosen for dynamic areas that require resilience without stiffness. For delicate refinement, particularly in the lips, we often select softer fillers such as Restylane Kysse or RHA 1.
The decision is never about brand loyalty. It’s about matching the material to your anatomy, tissue quality, and movement patterns. The right filler is the one that restores balance without adding heaviness.

Treatment is typically completed within 30–60 minutes depending on the number of areas. Most patients describe pressure more than pain. Lips tend to swell more than cheeks or jawline.
Structural Change, Natural Motion
After the procedure, you will notice an immediate improvement in the fullness and shape of your face. However, it's important to note that the full results may take a few days to appear as the swelling subsides. In addition, the longevity of the results varies depending on the type of filler used and the area treated. In general, results can last anywhere from six to eighteen months, with some fillers lasting up to two years. Our skilled team will work with you to choose the best filler and treatment plan to achieve your desired outcome. Dermal filler treatments typically require minimal downtime. However, there are a few simple rules patients should follow:
Most side effects are temporary: swelling, tenderness, and bruising. More serious complications are rare but real, which is why product choice and technique matter. We use anatomy-based planning, conservative dosing, and safety-forward placement. We also keep hyaluronidase available for HA fillers and educate you on what is normal versus what needs a prompt call.
Restraint & Anatomy First
Filler results are won or lost in the plan. At PCPS, Ver•ve Contour is built around judgment and restraint. We don’t chase trends or “one look.” We calibrate filler depth and product selection to the tissue in front of us—bone, fat, skin, and motion. Treatments are performed by experienced injectors with surgical oversight. You’ll understand what we’re doing, why we’re doing it, and what to expect as it settles.
All these are hyaluronic acid fillers, but they differ in texture and how they behave in motion. For example, Restylane includes options designed for lift, contour, and lips, while RHA is engineered to remain resilient in dynamic facial areas. We choose based on zone, tissue quality, and goals.
Not if it’s planned correctly. Our default is subtle structural support. We often stage treatment to avoid overcorrection and to keep your face looking like you. Avoiding frequent overfilling is essential. Fillers last much longer than the filler companies advertise.
Longevity varies by product, area, and metabolism. Many results last 12–18 months. We’ll estimate a maintenance cadence based on your plan.
Yes. Hyaluronic acid filler is used to refine contour, structure and proportion in specific areas. Over time, the internal structure of the filler softens and its contouring effect diminishes—even before it fully dissolves. Fat grafting is a minimally invasive procedure that transfers volume from one part of your body to another. Fat grafting is better suited when volume and improvement in tissue quality is needed.
Hyaluronic acid fillers can often be dissolved with hyaluronidase if needed, but that can also have side effects. We plan carefully to reduce the need, but it’s an important safety option.
