At the Pittsburgh Center for Plastic Surgery and Rejuvenation, we are committed to protecting your privacy and the confidentiality of your health information. This policy outlines how we may use and disclose your protected health information (PHI) in accordance with the Health Insurance Portability and Accountability Act (HIPAA).
We understand the importance of your health information and will protect it with the utmost care. Our staff is trained on the importance of safeguarding your information and adhering to HIPAA regulations.

Other uses and disclosures of your health information will be made only with your written authorization, which our office will specifically request from you. You may revoke this authorization at any time, except where we have already taken action in reliance on it.
We reserve the right to change this privacy policy. Any changes will be effective immediately upon posting the updated policy on our website and notifying you through other communication methods.
48 hours notice is required to cancel or reschedule appointments. Arrivals more than 15 minutes late may be considered a no-show. Missed appointments may require a consultation deposit for future bookings. Repeated no-shows may result in refusal of services.
Members agree to pay a fixed annual fee based on the selected membership. The annual fee will be charged to the Member’s designated payment method on the date of initiation of the membership.
Membership is on an annual basis unless canceled by the Member or terminated by the Center. The initial membership term is a minimum of twelve (12) months.
The Member may cancel their membership at any time by providing written notice (email or physical letter) to the Center. Any time a Member changes or upgrades their membership the initial start date will be the day of the membership change.
Membership fees are non-refundable. No refunds will be given for cancellation or modifications. Red Carpet Cash can only be redeemed for services or retail products while an Inner Circle Plus membership is active. Redemption is prohibited if the Member’s available Red Carpet Cash balance is less than fifty dollars ($50). Red Carpet Cash use cannot be combined with other offers or packages unless explicitly stated. After membership cancellation, Red Carpet Cash cannot be redeemed for services.
Members will retain their Red Carpet Cash after cancellation of membership for up to three (3) months (the “Grace Period”).
If a Member re-initiates their membership during the Retention Period, all previous Red Carpet Cash will be immediately available for redemption. After the Retention Period, Red Carpet Cash will expire and be cleared from the Member’s account. Expiration of Red Carpet Cash is irreversible.
