Blepharoplasty is an excellent option for healthy adults who want to improve the appearance of tired, drooping, or puffy eyelids. Ideal candidates are typically non-smokers in good general health who are bothered by excess eyelid skin, under-eye bags, or sagging tissue around the eyes. The procedure can enhance both aesthetic appearance and, in some cases, visual function if heavy upper lids obstruct the field of vision. 

However, eyelid surgery is not right for everyone. Certain medical and eye-related conditions may increase surgical risks or impact healing. Patients with glaucoma, dry eye syndrome, or other ocular surface disorders may not be suitable candidates, as surgery could worsen symptoms. Through this incision, excess skin, fat, and sometimes muscle are carefully removed or repositioned. In some cases, underlying structures are tightened to improve contour and support. The result is a smoother, more defined eyelid fold and a more rested, alert appearance without changing your eye shape. This procedure is commonly performed for both cosmetic reasons and functional concerns, including vision obstruction from heavy upper lids.

Lower Eyelid Surgery

Lower eyelid surgery addresses common concerns such as under-eye bags, skin laxity, and volume loss beneath the eyes. The procedure typically involves a discreet incision either just beneath the lash line or inside the eyelid, depending on your anatomy and goals. Excess fat can be removed or repositioned to reduce puffiness, while loose skin may be trimmed or tightened for a smoother contour. In some cases, the procedure is combined with fat grafting or dermal fillers to improve hollowness and restore a gentle transition between the lower lid and the cheek.

Transconjunctival Approach

The transconjunctival technique involves making an incision on the inside of the lower eyelid, leaving no visible external scar. Through this incision, fat pads can be either removed or redistributed to smooth contour irregularities or bulges. This approach is often used for younger patients with good skin tone but prominent fat pads and patients who require minimal skin removal.

Skin Incision (Subciliary) Approach

The subciliary technique uses a discreet incision placed just beneath the lash line. This allows the surgeon to remove or reposition fat and trim excess skin if needed. The incision typically heals well and becomes almost invisible over time. This approach is often preferred when there is redundant or crepey skin under the eyes or a more comprehensive rejuvenation is required. However, it can require a longer recovery time, and there are small risks of lid malposition in some anatomies.

Which Technique Is Right for You?
